Step 1: Getting Started

Step 2: Initial Consultation and Scope of Work Documentation

We will work closely with you to discuss the scope of your project and research requirements. After the initial consultation, we will prepare a Scope of Work (SOW) document that outlines the details of your project agreement and quotation for the cost of your project. Step 3: Scope of Work Verified and Confirmed

After you have reviewed and confirmed the Scope of Work (SOW) document, you can email or fax the appropriate order form and purchase order authorizing Affymetrix to initiate the project. Send all orders to our Affymetrix Customer Service Department:

Step 4: Send Samples

For services projects, please send your samples to our Affymetrix facility in Santa Clara. Please include a sample list, the Service Request Form, and the order form with your sample shipment. Please email an electronic copy of the sample list and the tracking number to the assigned scientist for your project.

Instructions for Sample Shipment

Serum, plasma, RNA, DNA, bodily fluids, tissue and tissue lysates, and cell culture samples:

Collect your samples in a 96-well plate or 1.5 mL micro-centrifuge tubes. Make sure your samples are sealed tightly and place your plate or tubes in a small box for further protection. When you are ready to ship, place the box in 8-10 lbs of dry ice and ship via FedEx overnight to Affymetrix for processing.

Samples should be sent between Mondays through Thursday. Samples should not be sent on a Friday as Affymetrix is closed over the weekend and sample integrity may be jeopardized. Client is to include a “Sample List Sheet” with the shipment and provide an electronic copy of that sample list sheet by email to the assigned Applications Scientist.

FFPE slides:

FFPE slides are placed in a plastic case and shipped at Room temperature. No dry ice is needed. FFPE samples can be shipped overnight, 2-day or ground Monday through Friday. Client is to provide a “Sample List Sheet” with the shipment and send an electronic copy of that sample list sheet by email to the assigned Application Scientist.

Any samples remaining may be returned to the client at their cost. Otherwise, they will be discarded 90 days after the completion of the assay service. This must be specified at the beginning of the assay service.

Step 5: Services Performed

Your project will be initiated and completed at our Santa Clara facility by our expert service staff. Depending upon the complexity of the project, we may send periodic updates as to the progress of the project. Please know that in order to start your project, we will need the authorization form/purchase order, a confirmation of the Scope of Work document, and the samples at our Santa Clara facility.

Step 6: Report Issued

Upon completion of the assay services, we will prepare a report that will include the data and summary of the services performed. To see a sample report, click here. We will also invoice your institution for the services as outlined in the proposal.
